Patients Served

Patients requiring monitoring/follow up for Chronic Kidney Disease (CKD), Eprex, Peritoneal Dialysis (PD) or Post Transplant.

Overview

This clinic initiates and monitors home PD. This includes teaching, home visits, telephone support and ongoing monitoring (testing, teaching, blood work, appointments) in the clinic as well as over the phone when blood work results necessitate changes in prescription.

This clinic has set targets to achieve the Provincial standard of 30-40% home therapy. Inpatient support is also within the mandate of this department. The Nephrology clinic assesses Chronic Kidney Disease patients (currently about 110 pts).

Regular appointments (3 months at a minimum), blood work follow up, Eprex teaching, dispensing and monitoring, and all inpatient support is part of this role. While this clinic is supported by the Renal budget, the staff themselves work CKD and PD in the same department.
